Allowance form is awarded based on your needs, not just on your medical condition. Many applications are turned down because people do not mention or are not clear about how their illness or disability affects their lives. Do not underestimate your needs when filling in the form.
Remember to include any help you need during the night too. You should also write on the diary how many times you need help with tasks like getting out of chairs. If you get help to fill in the form from your nearest Citizens Advice, remember to take your diary with you.
